After a referral is submitted, our team reviews the information and follows up within 1 business day. We may contact the referrer, the client, or both, depending on the consent provided and the information submitted.
Our team checks the referral for program fit, service area, contact information, and urgency.
A Point Healthcare Services team member contacts the referrer and, when appropriate and consent is confirmed, the client.
We review which program may be the best match: Health Home, FCS, DDA/DDCS, Peer Support, or another community resource.
If Point Healthcare Services is not the right program, we help identify a better next step whenever possible.
Referrers and applicants can use their confirmation reference number to check where the submission stands.
