Point Healthcare Services is a community-based care coordination, navigation, and support organization serving individuals and families throughout Washington State. We help people navigate healthcare, behavioral health, disability services, housing, employment, and community resources — with dedicated Care Coordinators who stay involved at every step.
We offer four core programs: Health Home Care Coordination, Foundational Community Supports (FCS), Developmental Disability Services (DDA/DDCS), and Peer Support Services. Each program is designed to help individuals with specific needs access the right services and support.
We stay involved. Unlike a standard referral process where a case is closed after a referral is made, our Care Coordinators remain actively involved — following up, advocating, and making sure the people we serve actually receive the support they need. We don't just connect you to services; we walk alongside you.
Eligibility varies by program. In general, most of our programs serve individuals who are enrolled in Medicaid (Apple Health) and have qualifying conditions such as two or more chronic health conditions, a serious mental illness, housing instability, or other complex needs. We currently serve individuals in King, Pierce, Snohomish, Kitsap, Thurston, Jefferson, Chelan, and Clallam Counties. We meet clients in their homes, hospitals, facilities, and communities — wherever they are.

Health Home Care Coordination is a Medicaid-funded program that helps individuals with complex health conditions navigate healthcare, behavioral health, long-term supports, and community resources. Our Care Coordinators develop personalized care plans and provide ongoing support.
Your Care Coordinator is your dedicated point of contact. They help you develop a care plan, coordinate appointments and services, follow up with providers, connect you to community resources, support care transitions, and advocate for your needs — staying involved throughout your journey.
You may qualify if you are enrolled in Medicaid (Apple Health), have two or more chronic conditions or one serious mental illness, and live in one of our service counties. Contact us to learn more about your eligibility.
Yes. Our Care Coordinators can meet with you in your home, at a clinic, in the hospital, or in your community — wherever works best for you. We believe in meeting people where they are.
There is no cost to eligible Medicaid (Apple Health) members. Health Home Care Coordination is funded through Washington State Medicaid and is provided at no cost to qualifying individuals.
FCS is a Medicaid-funded program that helps eligible individuals find and maintain stable housing and connect with employment opportunities. It addresses the social factors that most directly impact health and well-being.
Yes. Through FCS, we provide supported housing assistance, help with rental applications, connection to housing resources and emergency support, and navigation of available programs to help you find and maintain stable housing.
Yes. FCS includes employment support services including job coaching, help identifying employment opportunities, benefits navigation, and connection to employment programs and community resources.
Referrals are welcome from anyone — physicians, hospitals, clinics, managed care organizations, social workers, case managers, family members, and individuals may all refer themselves or someone they care about.

Our team will review the referral and reach out to the individual or their representative to discuss eligibility, next steps, and how we can help. We work to make the intake process as smooth as possible.
