Health Home care coordination is a Medicaid-funded program that helps individuals with complex health conditions navigate healthcare, behavioral health, long-term supports, and community resources. Our care coordinators provide personalized support, develop individualized care plans, and work alongside the people we serve to help them access the right services at the right time.
