Foundational Community Supports (FCS) is a Medicaid-funded program that helps eligible individuals find and maintain stable housing and connect with employment opportunities. We work alongside individuals to address the social determinants of health that most directly impact their ability to thrive.

Who May Qualify

Eligible for Medicaid (Apple Health)
At risk of homelessness, or experiencing housing instability
Or seeking to enter or maintain employment

What We Provide

Supported housing assistance
Employment support and job coaching
Rental assistance navigation
Connection to housing resources and emergency support
Benefits navigation and planning
Community resource referrals
