- Medicaid
- A joint federal and state program that provides health coverage to eligible people with limited income, people with disabilities, and others.
- Care Coordinator

- Care Plan
- A personalized, written plan built with you that lists your goals, the services you'll use, and the steps to get there.
- Health Home
- A Medicaid-funded care coordination program for people with complex health needs — not a place, but a way of organizing your care around you.
- FCS (Foundational Community Supports)
- A Medicaid-funded program that helps eligible people find and keep stable housing and employment.
- DDA (Developmental Disabilities Administration)
- The Washington State agency that provides services and supports to people with intellectual and developmental disabilities.
- MCO (Managed Care Organization)

- Peer Support Specialist
- A trained professional who uses their own lived experience with recovery to support others on a similar journey.
- Referral
- A request — from a provider, family member, or yourself — asking us to connect someone with one of our programs.
- Intake
- The first conversation after a referral, where we confirm eligibility, understand your situation, and get you enrolled.
- PHI (Protected Health Information)
- Personal health details protected by law (HIPAA). We never ask you to send PHI through this website.
- Social Determinants of Health
- The everyday conditions — housing, food, transportation, employment, community — that shape health as much as medical care does.
- Care Transition
- Moving between care settings, like leaving a hospital for home. We coordinate these moments so nothing falls through the cracks.
