We serve Medicaid- and Medicare-eligible individuals who may be managing chronic medical conditions, behavioral health needs, disabilities, housing instability, or other barriers that affect their health and well-being.
Our Case Managers provide culturally responsive, person-centered support that connects clients with healthcare providers, community resources, housing assistance, employment services, disability programs, and other essential supports.
The Case Manager works directly with clients to identify their needs, establish meaningful goals, coordinate services, and help remove barriers to care.
This position requires strong communication, organization, follow-through, and documentation skills. The Case Manager manages an assigned caseload, conducts outreach, completes client intake appointments, meets clients in the community, develops Health Action Plans, completes required assessments, coordinates referrals, and documents all services within required timelines.
This is a nonclinical case management position. The Case Manager provides care coordination, education, advocacy, follow-up, and resource-navigation support while collaborating with healthcare providers, caregivers, community organizations, and other members of the client's care team.
